چکیده

Objective: Intracranial dural arteriovenous fistulas (AVFs) account for 10%-15% of all intracranial arteriovenous lesions. Some dural AVFs produce ocular symptoms include proptosis, diplopia, episcleral venous engorgement, periorbital swelling, extraocular muscle limitation, visual field defect, and papillaedema. Materials and Methods: We reviewed four patients who had dural AVFs with ophthalmic manifestations in the past three years (2000 November to 2003 October.) The diagnosis of dural AVF was confirmed by image studies, such as magnetic resonance imaging (MRI), magnetic resonance angiography (MRA) and cerebral angiography. Results: There were three men and one woman. According to Cognard’s classification, there were four types: type I, type II a, type II a+b, and type III. Symptoms at initial presentation included headache, diplopia, proptosis, episcleral vein engorgement, periorbital swelling, visual field defect, and papillaedema. Three patients received transarterial embolization (TAE) in our hospital. Improvement in ophthalmic symptoms and signs was noted after TAE treatment, although multiple interventions were required. Conclusions: Dural AVFs can produce various ocular symptoms and signs at their initial manifestation. The symptoms are believed to reflect venous hypertension in the superior sagital sinus, resulting from the shunted flow, which interferes with normal venous drainage. Transarterial embolization can be an effective treatment to close the fistula, restore sinus function and improve ocular symptoms. (Tzu Chi Med J 2005; 17:93-97)

Spinal dural arteriovenous fistula : a case report

Background: Spinal dural AVF is the most common type of spinal vascular malformation. However, presenting symptoms differ according to site of spinal involvement. This study described a case of arteriovenous malformation with paraparesis and incontinence. Case Presentation: Diagnosis of patient was confirmed by clinical and imaging examination using magnetic resonance image and ruling out ot...

Transvenous Coil Embolization for Dural Arteriovenous Fistulas of the Ophthalmic Sheath: Report of Two Cases and Review of the Literature

We present two patients with a dural arteriovenous fistula (dAVF) of the ophthalmic sheath who developed progressive exophthalmos, conjunctival chemosis, and visual loss. These symptoms mimic those of cavernous sinus dAVFs. Dural AVFs of the ophthalmic sheath are extremely rare and their clinical management is controversial. We successfully treated these two patients by transvenous coil emboliz...

Trigeminal Cardiac Reflex Caused by Onyx Embolization of Intracranial Dural Arteriovenous Fistula.

Trigeminocardiac reflex (TCR) is a reflexive response of bradycardia, hypotension and gastric hypermotility which is observed upon mechanical stimulation in the distribution of the trigeminal nerve. Previous articles have described TCR during intracranial operations, ophthalmic surgery, microcompression of the trigeminal ganglion and radiofrequency lesioning of the trigeminal ganglion. TCR may ...

Transvenous Embolization of Cavernous Sinus Dural Arteriovenous Fistula Using the Direct Superior Ophthalmic Vein Approach: A Case Report

Transvenous coil embolization has been successfully applied for the treatment of cavernous sinus dural arteriovenous fistula (CSDAVF). Unfortunately, the technique cannot be applied in cases of poor or absent inferior petrosal sinus or facial venous access route to the fistula. Recently, we experienced a successful embolization using direct superior ophthalmic vein approach in cases of CSDAVF w...

Dural Arteriovenous Fistula of the Transverse and Sigmoid Sinus Manifesting Ascending Dysesthesia: Case Report and Literature Review

Cases involving intracranial dural arteriovenous fistulas (AVFs) with spinal perimedullary venous drainage exhibit variable presentations, which results in delayed diagnoses. We describe a case of a 66-year-old female with a transverse-sigmoid sinus dural AVF with spinal perimedullary venous drainage who developed dysesthesia and hypalgesia that ascended from the peripheral lower extremities. S...
